Designed for home blood pressure management, this solution allowed patients to log readings over a 7-day or 14-day monitoring period through a digital app. These readings then feed directly into the patient's GP health record, eliminating the need for manual data entry. Additionally, the pathway supported assessments for undiagnosed hypertension, diagnostic reviews, and medication titration when required.
