About Digital Care Home
Digital Care Home South is a digitally enabled programme designed to improve the quality of care provided to residents within care homes across South Cumbria.
All care homes in South Cumbria will be equipped with a tablet device and observation equipment.
The project is intended to provide more consistent communication between care homes and OOH clinicians, reduce avoidable telephone based referrals, support care home staff and improve pathways meaning urgent cases are seen within the correct timeframe.
The overarching outcome of the programme is to improve care quality for care home residents. The existing process has relied heavily on telephone communication. The quality and completeness of information available to the receiving clinical team can therefore vary, and clinicians may need to obtain basic information during the telephone call before determining the most appropriate response.
Digital Care Home South introduces a structured digital SBAR referral form.
